Prepare for Your Visit

What should I bring to my immigration medical exam?

Straight answers from Dr. Lishan Kassa, MD, a USCIS-designated civil surgeon in Alexandria, VA.

Quick Answer

Bring a valid government-issued photo ID (passport or driver's license), your vaccination records, any prior medical records including TB test results, a copy of your immigration paperwork, and a list of current medications.

The Details

Photo ID. A passport or driver's license. We record your ID on Form I-693.

Vaccination records. Bring every record you have, including foreign records with an English translation. The more records we can enter, the fewer vaccines you may need. If you have no records, tell us when you schedule.

Medical records and paperwork. Prior medical records including TB test results, a copy of your immigration paperwork or Form I-693, and a list of current medications.

Payment. The exam fee is due at your visit. Lab testing and vaccines, if needed, are billed separately.

Please do not sign Form I-693 yourself. Wait until the civil surgeon tells you to.
