Form I-693 is the official USCIS Report of Medical Examination and Vaccination Record. It must be completed by a designated civil surgeon and submitted in a sealed envelope as part of your Green Card application or adjustment of status.

Anyone applying for a Green Card (permanent residence) or adjustment of status in the United States is required to complete an immigration medical exam performed by a USCIS-designated civil surgeon.

If all lab results are normal and no follow-up is needed, the completed Form I-693 in a sealed envelope is typically ready within a few days to one week after your exam.

No. The completed Form I-693 must remain in the sealed envelope. Opening it will invalidate the form, and you will need a new exam. Submit the sealed envelope directly to USCIS.

TB screening is required by USCIS for nearly all applicants age 2 and older. Additional tests, such as syphilis screening, are generally required for ages 18–44 and may apply outside that range if clinically indicated, per CDC guidelines. Samples are collected at your visit and sent to Sunrise Medical Laboratories, a Sonic Healthcare clinical laboratory, or to Labcorp or Quest Diagnostics as needed, for processing. Yes. Children applying for Green Cards or adjustment of status also need the exam; requirements may differ by age. The child exam is $250. We see patients of all ages.

Generally, fasting is not required. However, if blood work is being done, you may be asked to fast. We will provide specific instructions when you schedule.
