Information we collect
- When you book or contact us: your name, email address, phone number, appointment details, the number of people being examined, and any notes you provide. Online booking runs through our scheduling provider (Lark).
- Request an Appointment form: this form opens your own email app with a pre-filled message. Nothing is sent to us until you send that email.
- At your visit: identification, vaccination and prior medical records, and health information needed to perform your immigration medical exam and complete Form I-693.
- Technical data: like most websites, our hosting provider may log standard information such as IP address, browser type and pages requested.
How we use it
- To schedule, confirm and remind you of appointments, and to answer your questions.
- To perform your exam, review your records, order required tests and vaccines, and complete your sealed Form I-693.
- To tell you when your form is ready for pickup.
- To keep the site secure and working properly.
Health information
Health information we create or receive while providing your care is protected under HIPAA and other applicable law. Please ask us for our Notice of Privacy Practices. Please do not send sensitive medical details through the website; use the phone or your visit instead. Records you email us are handled as part of your file.
Who we share it with
- Laboratories and pharmacies you choose, to the extent needed to fill the lab orders and vaccine prescriptions we give you.
- USCIS, through the sealed Form I-693 we give to you for submission with your application.
- Service providers that help us run the practice, such as our scheduling and email provider (Lark) and web hosting provider, under their own privacy terms.
- Others when required by law or with your written permission.
We do not sell your personal information.
Cookies and third-party content
We do not use advertising or analytics trackers on this site. The embedded map (Google Maps), fonts (Google Fonts) and icon library may receive your IP address and set their own cookies under their own policies. Links to other sites, including our Google reviews page and the booking system, are governed by those sites' policies.
Security and retention
We use reasonable administrative and technical safeguards, but no method of transmission or storage is completely secure. We keep records as long as required by medical-record and immigration rules and applicable law.

This site is intended for adults. Exams for children are arranged by a parent or legal guardian.
